Tournament Entry Procedures (US Mail)
Follow the steps
Below to manually enter this tournament and mail out your entry from
and check to Super Series Indiana.
If you want to expedite the process, scroll down
below for the online way to enter.
1. Download the
tournament entry from below, fill out information
and mail to the tournament director with your entry fee and
Spring/Summer 2006 Roster Registration.

2. Register your team
with the Super Series National Office
(Cost is $ 35.00 - payable via
Mastercard or Visa and is good for the entire Spring/Summer 2006
season.)

birth dates, addresses and phone numbers for the Super Series
Roster.

Make Checks Payable to:

This is the fastest way
possible to enter your team into our tournament.
You will still have to mail in your tournament entry payment,
however, this form will email your contact information and "intent" to participate, and hold your spot for 5 business days
until your check arrives via the US Postal Service.

IMPORTANT NOTE TO ALL TEAMS
Entry fees must
be paid in advance. No teams are entered in our events until
their completed entry form, entry fee and national
registration/roster are received.
Make all entry
fees payable to Super Series
MidAmerica, Inc.
Don't forget that
by enclosing a copy of your official Super Series
team Roster/Registration with your entry form and entry fee,
you may deduct $25.00 from the initial stated entry fee listed.
(Example: If the entry fee for an event (per team)
is $425.00, you may deduct $25.00 because your team is already
registered with the national organization, so the actual cost for
the tournament would be $400.00)
